If your plan includes out-of-network benefits, your employer and you have already invested in access to a higher level of care. Insurance & Fertility Testing
- Many fertility tests recommended during preconception care are not typically covered by insurance, though coverage varies by plan and carrier.
- Our team will always verify your benefits before testing is performed, explain any potential out-of-pocket costs, and help you maximize any available insurance coverage.
Cash-Pay Options
If we do not participate with your insurance plan, we are happy to provide transparent cash-pay pricing.
Many patients choose Viva Eve’s cash-pay wellness services because of the comprehensive nature of our care. Depending on the visit type, services may include:
- Comprehensive wellness consultations
- Bloodwork and laboratory testing
- Sonograms and diagnostic imaging
- Physical examinations
- Personalized treatment planning
- Extended appointment times with your medical provider
For approximately $600, patients receive a comprehensive wellness experience designed to provide greater access, individualized attention, and preventive care.

Some laboratory tests and diagnostic services may incur additional charges. If additional testing is recommended, your provider will discuss the reasons for the recommendation and provide available pricing information before any testing is performed.

Payment Policies for Aesthetic & Wellness Services
Cosmetic Dermatology
Cosmetic Dermatology services are cash-pay and payment is due at the time of service.

Leg Vein Treatments
Asclera® cosmetic leg vein treatments are generally not covered by insurance and are cash-pay services.
However, deeper vein treatments and medically necessary varicose vein procedures are often eligible for insurance coverage. Our Vein Specialists will verify your benefits and discuss coverage before treatment begins.
Nutrition Services
Elective nutrition programs with Tammy Jordan, RD are cash-pay services.
Please note: Nutrition visits with Tammy Jordan are included for Viva Eve obstetric patients throughout pregnancy. OB patients do not incur additional cash-pay fees for these included visits.
